Provides assessment of chemical use problems, creates and monitors individualized treatment plans, provides family counseling and chemical health counseling services to individuals with chemical abuse problems; and performs related duties as assigned. Works with patients 18 years and older.

MINIMUM QUALIFICATIONS:

Education:

Graduation from an accredited chemical dependency counselor training program and an Associate degree. A Bachelors degree is strongly preferred.
